University of Iowa Summer Sports Camps: Building Future Champions

MEDICAL RELEASE FORM: We recommend you fax (319.335.7970), email (sportcamps@hawkeyesports.com), or mail (203 Field House, Iowa City, IA 52242) your medical release form in advance. If we have not received your medical release form the first day of camp, we can't allow you to participate until we receive it. A school sports physical is acceptable (within one year of the camp date), in addition to the right hand side of the medical release form, filled out by a parent or guardian.

If you can't locate the link to pay your balance online, call 319.335.7961 and we will send the email again. Please don't fill out a new application.

We receive many calls asking if we accept out of state campers, and if there is a price difference. Resident refers to your housing arrangements while you attend camp, not your home address and the same prices apply to all campers. A resident sleeps in Hillcrest dormitory over night with us and a commuter has made private sleeping arrangements other than Hillcrest dormitory. If you attend camp with us as a commuter, your lunch and dinner are included in your fee.
