When we receive your application, we will mail you a confirmation letter and medical authorization release form. Each specific sport has a group of downloadable forms pertaining to that sport. Please print a copy and retain for your personal records.
If you do not the capability to download and print your forms, please contact the Sports Camps office and we will make sure you receive them.
NOTE:
Football, Boys Basketball, and Volleyball have a medical authorization and release form included in their brochures. If you need an additional medical authorization and release form, you may download one from any of the below sports.
