What if I can’t get a medical release or school physical?

A medical release MUST be signed by a doctor or a copy of your school sports physical within one year of the camp date to attend any of our camps. If you come to camp without a medical release, you can contact a doctor and have them fax us a copy or go to a local UI QUICK CARE and have a sports physical performed. There are NO EXCEPTIONS. If you choose to have a doctor fax us a copy, we can’t allow your child to participate in any functions until this medical release is provided to the trainer.

When is my medical release due?

You don’t have to mail your medical release with your original application. However, we would prefer that you send your completed form before you check in the first day of camp, and you can do this via fax, scanning and sending an email or the US mail. There are so many things to remember when getting ready for camp, and if you would happen to over look the form and forget it, we can’t allow you participate until we have it.
